全３回：息子を例に発達障害を考える【３】

⚠︎：発達障害は先天的な脳の発達の偏りなので、親の躾や環境、また、本人のせいでもありません。発達障害をややこしくしているのは、無理解、無知からくる不適切な対応などでさまざまな神経症や精神疾患を併発したことによる二次障害といえます。

最終回の【３】では、軽く視知覚認知機能や視空間認知機能に触れながら、それらの弱さからくる困り感を、日常起こりやすい事例を交えながらお話します。

彼らは無意識に物を置きますが、次々に物を重ね置き、無秩序にあちこち置くので、下にあった物が埋もれ見えなくなります。

その見えなくなった物は【ない】もしくは【失くなった】になります。

例えば消しゴムの上にポイッと請求書、その上に読みかけの本、その上に脱いだ服といった具合です。こうなると、消しゴムはもちろん、請求書も読みかけの本も忘却の彼方へスッポリと抜け去ってしまいます。

ＡＳＤは散らかった中から必要な消しゴムをすぐに取り出せても、ＡＤＨＤは消しゴムを探し出すことはできません。なぜなら『失くなった』（視界から消えた）からです。引っ掻き回して消しゴムを探すうちに、読みかけの本を見つけて読み始め、本来の“消しゴムを探す”目的はスッポリ抜け落ち、これまた忘却の彼方へ・・・。これは『ＡＤＨＤあるある』のようです。

散らかっているという認識が薄い時は、自分にそのような無意識に思い込みや視知覚認知フィルターがかかっている状態なので、肉眼ではなくデジタルカメラやタブレットで撮影し、それを通して見ることで自分本意な視知覚認知フィルターを外し、多少客観的に見ることができます。画像データを通して客観的視点で見てみる、という感じでしょうか。

頑張って片付けた時に、部屋の全体像や片付けた場所、片付け方を撮り、それを見える場所に貼って見本にしながら片付けるやり方も効果があります。

話は戻りますが、【２】で息子の独特な認知特性が人間関係の躓きを生んでいるという話をしましたが、学習面における困り感の原因のひとつである視知覚認知についてお話します。

視知覚認知機能とは私たちが普段＜見える、見えない、目が遠い＞と言っている視力のことではなく、

【 見 る 力 】

のことで、

  • ふたつの目が負担なく機能しているか？
  • 見るものを正しく目で捉えられているか？
  • 目の動きに無理はないか？
  • 目から入った情報を正しく認識、処理、出力（書く、読む、写す、四肢を動かす）できているか？

などの機能です。これに目からの情報を処理して空間全体をイメージする機能、視空間認知が加わります。この視知覚認知機能に不具合があると、

・文字を読むのに時間がかかる。
・字が覚えられない。
・左右認識が苦手。
・探し物を見つけられない。
・長時間集中して作業できない。
・本を読むのに、行を飛ばしたり同じ場所を読む。
・図形理解が苦手。
・板書を写せない。
・ケアレスミスが多い。
・手足の連動を必要とする運動が苦手。
（キャッチボール、自転車、テニス、縄跳びなど）
・手先が不器用で、手先を使う作業が上手くできない。
（折り紙、コンパス、裁縫作業など）
・服のボタンのかけ違いが多い。
・不注意であちこちにぶつけたり挟む。
・よくつまずく。

などといった、ＡＤＨＤの“不注意”からくる特徴にも被る困り感が表れます。

視覚なのに運動？と思いますよね。視覚と身体感覚には深い関係があり、手足の連動にも大きく関わってくるのです。

また視空間認知は

・形や色の認識。
・形や方向に関係なく同じ形を「同じ」と把握できる。
・対象にするものと背景の区別。
・対象にするものとの距離を測る。

といった機能です。

これに加え、発達障害では発達性協調運動障害というものもあり、微細運動（はみ出さずに塗り絵をする、コンパスを上手に使う、箸を上手く使う）や、粗大運動（スキップや跳び箱、大縄跳び）に問題を持つケースも多く見られます。ザックリ言うと、

とても不器用で大雑把、動作が雑

に見えます。実際息子はしょっちゅう物を落としたり、コップやお椀をひっくり返しますし、一つ一つの動作が雑で、脱いだ衣類をきちんとハンガーに掛けることや畳むことも苦手です。またＡＤＨＤには脳機能の中にある実行機能と報酬系という部分の機能低下が見られるので、先送りグセやマルチタスクが苦手という特徴で表れます。

実行機能とは、

  • 行動を分析、組み立て、優先順位をつけて遂行する能力。
  • 一時的に記憶を保持しながら他の作業をおこなう（ワーキングメモリー）。
  • 感情がモチベーション、覚醒の自己調整能力。
  • 他者との会話の内在化

です。これらの機能低下で行動制御に不具合が起こり、結果、

何から行動すればよいのかわからなくなり、順序立てた行動が取れず、すぐに取り掛かれない。

記憶（情報）を保持しながら他の作業ができないことで、他に注意が向くと前のことを忘れてしまうといった症状が表れる（消しゴムの例）。

感情のコントロールが難しく、イライラ感を持ちやすく爆発しやすかったり、思ったことをすぐに口に（行動に移す）出してしまうなどの症状としても表れる。

報酬系とは、

満足感、充実感や達成感を得る系で、この働きの低下で充足感が不十分になり、報酬の強化が十分におこなわれず、衝動的な行動や注意を拡散し、気を紛らわせるなどの代償行動として、多動性や不注意が表れる。

この、神経伝達物質ドーパミンとノルアドレナリンが関わる部分に対しては薬物療法が有効とされ、息子も何種類かの薬を服用しています。

これは飽くまでも息子の本人談ですが、薬効について聞き取りをしたところ、服薬するしないでは“集中や衝動性”には自覚できる大きな変化があるそうです。ただ、不注意にはあまり変化がなかったようでした。

主治医の判断と、作用副作用の出現の仕方も個人差が大きく、薬が万能というわけではありません。やはり、どのような薬であっても服用に関しては個々が慎重に考えるべきだと思っていますので、あえて服用中の薬の内容については言及を避けます。

避けたくて避けても生活に影響がない困り感ならば、敢えて避けるというのもライフハックと言えますが、社会生活、日常生活に影響が出やすい困り感には、自分にマッチした対応策があるとＱＯＬにも大きく関わってきます。

そのためには苦手や困り感に目を向け、必要とあらば適切な医療機関の受診、関係機関との連携を取り、自分に合った対応策を考えていく（一緒に考えてもらう）ことが大切だと思っています。
